Do I have to have exactly 5 cards in the graveyard to activate it? Or can I activate it with less/none?

You must have, AT LEAST, 5 monsters in the graveyard to activate it.

To follow up, all 5 of them must go back into the deck / extra deck at the same time or it does not resolve.

Pot of avarice sucks when you draw it early in the game.
Theres also another card like it, Pot of Benevolence ; Select 2 cards from the Graveyard(s), and shuffle them into their owner's Deck(s). After activation, remove this card from play instead of sending it to the Graveyard. You can only activate 1 "Pot of Benevolence" per turn.

Advantages : Add 2 cards in the graveyard to the deck, any cards.
Disadvantages : No draw and only 2 cards.

Benevolence is terrible, end of story.

PoA should really only be run in decks that have a burning need to recycle resources these days.
